Offered in partnership with Advisors on Aging, this service includes:

  • Comprehensive assessment of care needs
  • Home safety assessment
  • Counseling about long-term care options
  • Facilitating family meetings
  • Acting as liaison with families living long-distance
  • Assistance with placement
  • Help with planning for a discharge from a skilled nursing facility or hospital
  • Medical care coordination and advocacy
  • Referrals to MDs, transportation, daytime care, and other services
  • Educational resources
  • Home safety assessment
  • Arranging short-term respite
  • Providing assistance in applying for CBAS and other public benefits
  • Escort to medical appointments

Our care coordinators will also provide referrals to:

  • Geriatric specialists
  • Physicians
  • Daytime care
  • Emergency alert systems
  • Attorneys
  • Financial advisors
  • Hospice and other home and community-based service
